No. ISKh-27478 from November 29, 2025 On November 25, 2025, at 4:21 p.m., upon arrival of train No. 2702 with VL80S electric locomotive No. 2086/2096, registered at TChE Liski, under the control of the locomotive crew of TChE Timashevskaya T S-KAV on track No. 4 of the station. Bakanskaya was allowed to pass exit signal Ch4, subsequently cutting switch No. 9. Circumstances: A preliminary investigation revealed that train No. 2702 was accepted onto track No. 4 at Bakanskaya station before exit signal Ch4 with a prohibiting signal to clear behind a commuter train departing from track No. 2, of which the DSP informed the driver of train No. 2702. While traveling along track No. 4, the locomotive crew passed route signal ChM4 at 38 km/h, located 245 meters before exit signal Ch4, despite the requirement of the Local Brake Control Instructions of no more than 20 km/h. After passing the ChM4 route signal with a yellow indicator and the KPD-3 indicator unit displaying a red-yellow indicator, the SAUT CM brake control system applied the brakes, illuminating the "Release Prohibited" lamp due to the actual travel speed exceeding the SAUT-CM speed limit. The driver switched the SAUT to ALS mode and released the brakes by setting the driver's control valve handle, condition No. 394, to position 1. As the train continued to travel, the locomotive crew, seeing the prohibiting indicator of the Ch4 exit signal in the immediate vicinity, applied emergency braking, but due to the short distance, allowed three sections of the electric locomotive and two cars to pass. Reasons: The primary reason for the permitted passage of a red light at Bakanskaya station was exceeding the speed limit for a section shorter than the braking distance, interfering with a properly functioning locomotive safety device, and ignorance of the station's TRA. A secondary reason was the appointment of a locomotive engineer without passing a test on knowledge of the TRA of the stations served in the AS GRAT system. Resolution: Based on the above, all T, TChE: 1. Organize the inclusion of the circumstances and diagram of the red light (attached) committed at Bakanskaya station in the pre-trip briefing of locomotive crews. 2. Conduct a review in December 2025 of the personal files of engineers appointed to positions in 2025 for the presence of test reports on the TRA of the stations served. 3. Include in the pre-trip briefing for locomotive crews an excerpt from the Local Brake Control Instructions listing stations with traffic lights that delimit sections shorter than the braking distance.
