Switch Break at Kurbakinskaya Station, Moscow Railway, August 14, 2025


On August 14, 2025, at 2:01 a.m., as a result of the unauthorized movement of a car from track #4 at Kurbakinskaya Station, Moscow Railway, switch #43 was broken.
The investigation revealed that on August 13, 2025,
at 11:30 p.m., a formed train was moved from a private track to track #5 at Kurbakinskaya Station. Following the technical inspection, the car inspector/repairman stated
that the fourth car needed to be relocated to eliminate the difference in the coupler height after the locomotive was coupled.
On August 14, 2025, at 1:54 a.m., a shunting train (a locomotive and four cars) was moved to a free receiving and departure track No. 4.
At 1:57 a.m., the train conductor (with two months' experience) reported to the dispatcher about placing one brake shoe on each side of the car to be left on track No. 4. However, in fact, in violation of paragraph 1 of Appendix No. 12 to the IDP, the car was not secured.
After making a fictitious dispatcher report about securing the car to be left, the dispatcher uncoupled the cars and instructed the driver to leave track No. 4.
As a result, at 2:01 a.m. A car left on track No. 4 began to move spontaneously, subsequently opening switch No. 43.
It should be noted that a similar traffic safety incident had previously occurred at this station in December 2023.
This year, five traffic safety violations related to the unauthorized movement of rolling stock have been committed at the expense of train organizers.
In order to minimize the risk of similar traffic safety violations, I hereby order:
1. D, DCS, DS, within three days of receiving this telegram, to organize a briefing for employees involved in train operations:
a) regarding the causes and circumstances of the incident of opening switch No. 43 during shunting operations, which occurred on August 14, 2025, at Kurbakinskaya station on the Moscow Railway;
b) on compliance with the requirements of paragraph 1 of Appendix No. 12 to the IDP regarding leaving rolling stock without a locomotive only after the rolling stock has been secured in accordance with the procedure established by the station's TRA. Inform employees of the legal requirements regarding liability, including criminal penalties, for leaving cars unsecured.
2. From August 18 to September 18, 2025, the Department of Moscow Railroad Construction (DMOSK) shall ensure compliance with the requirements of Table A of Appendix No. 12 of the Regulations for the Functioning and Development of the SMBD, approved by Order No. CD-1/r of January 1, 2025. 3. All D, DTS, DNC, and DS shall ensure that inspections are conducted in August–September 2025 according to the developed schedule, in a process-based monitoring mode. This inspection shall include employees responsible for the assignment of rolling stock who are in risk group "2" or who have less than one year of service. This inspection shall address the following issues:
compliance with the procedures and standards for the assignment of rolling stock;
ensuring control over the assignment of rolling stock by shunting supervisors.
4. DTS shall organize, by October 30, 2025, an unscheduled knowledge test for train conductors in "Risk Group 2" in accordance with Appendix No. 15 to the Regulation, approved by Order No. CD-248/r of November 25, 2019, in accordance with the requirements of Appendix No. 12 to the IDP. 5. I assign control over the execution of this telegram to D.
