The work carried out at the Far Eastern Railway Transport Department (DVOST) aimed at ensuring compliance with mandatory traffic safety requirements did not produce the expected results.

The lack of systematic work to eliminate risk factors that influence the occurrence of traffic safety violations led to further instances of passing traffic lights with a prohibiting signal.

On October 13, 2025, at 6:51 a.m. Moscow time, at the Ogoron station, While freight train No. 2918 was traveling on track two at Ogoron Station, locomotive ZTE10UK No. 0042, registered at TChE Novy Urgal, driven by a locomotive crew dispatched from TChE Kotlas SEV T, passed the ChM2A route signal with a prohibiting signal.

Circumstances:

The investigation established that freight train No. 2918 was scheduled to travel by the DNC, stopping on track two at Ogoron Station to allow freight train No. 2921 to pass on track one. The Ogoron Station DSP then informed the driver of the receiving route, who confirmed the information.

Upon arrival at the station, the train passed the ChM2A signal with a prohibiting signal because the locomotive crew mistakenly perceived the ChM2A route signal as a shunting signal.

The train was stopped by service braking after traveling 81 meters.

The distance between the exit signal CM2 and the route signal CM2A is 276 meters, which is less than the braking distance.

Causes:

The immediate cause was distraction and failure to monitor the traffic signals along the route.

A contributing cause was failure to comply with the requirements for training assigned locomotive crews for independent work regarding violations in studying the profiles and TPAs ​​of the stations served by the sections, as well as failure to provide assigned workers with the necessary documentation (extracts from local instructions, orders on speeds, etc.).

1.1. Organize and conduct, in accordance with the established procedure, an unscheduled briefing on the admitted traffic safety violations.

1.2. As part of the Safety Culture Days, communicate the circumstances and root causes of these traffic safety violations to the TChE teams.

1.3. By December 26, 2025, verify the ordinates of traffic lights to determine whether the distance between them is less than the braking distance, with subsequent verification against the data entered in the Local Brake Control Instructions. Organize the updating of these instructions, if necessary.

1.4. By January 30, 2026, submit the results of the ordinate verification of traffic lights with distances less than the braking distance to the DI and JSC NIIAS to update the relevant regulatory framework.

1.5. Establish a procedure for issuing a list of traffic lights located at a distance less than the braking distance to locomotive crew members by section, in accordance with the issued reports.

1.6. In November-December 2025, include a task to inspect locomotives for unauthorized installation of jumpers and the use of "dummy" TSKBM safety devices in the targeted inspection plan.

1.7. Chief engineers of locomotive operating depots are to check the availability and relevance of organizational documents at the TChE establishing the procedure for using the TSKBM when permitting locomotive crews to work, and the actual performance of periodic checks of the PNC devices and TSKBM wearable parts by employees before trips (shifts), as well as the availability of the TSKBM-N transition stock at the TChD.

2. T DVOST, in accordance with the procedure established by JSC Russian Railways, is to conduct an investigation into the traffic safety violation that occurred at Khabarovsk II station, and to impose penalties on the employees involved.

3. Submit a progress report by February 10, 2026, via EASD to "Anton Yuryevich Pakhomenko."
