Report
on the admitted traffic safety violation at Zyurzya station of the Gorky Railway

On August 27, 2025, at 7:58 p.m., at the Zyurzya railway station of the Izhevsk territorial administration of the Gorky Railway (district, 2nd class), a single shunting locomotive, ChMEZ series No. 5957, passed the prohibiting signal of exit signal 47, then departed onto the receiving route of train No. 2253, resulting in a collision.
Circumstances:
The shunting plan called for the transfer of a single ChMEZ shunting locomotive #5957 from track #4 to track #7 (via the even-numbered station gate).
At the same time, it was necessary to accommodate odd-numbered freight train #2253 on track #9 at the odd-numbered station gate (for a locomotive crew change).
After receiving train #2253 on track #9, the shunting locomotive was scheduled to uncouple a faulty car from train #2057 on track #3 (via the odd-numbered station gate).
At 7:46 PM, DSP Z.F. Andarzyanova brought A.A. Kuznetsov to the shunting station. The shunting plan for moving the shunting locomotive from track #4 to track #7 before red light 47 was completed.
TChM Kuznetsov A.A. repeated the completed shunting plan in full, and the DSP confirmed the accuracy of his understanding.
At 7:47 PM, the route was prepared and shunting movements began from track #4 in the direction of the first main track beyond signal M4.
At 7:50 PM, the route was prepared and shunting movements began from signal M4 onto clear track #7.
At 7:53 PM, the shunting locomotive entered track #7 and proceeded to the EC post (for the boarding of train dispatcher V.V. Nosov).
After the shunting locomotive entered track #7, at 7:53 PM, Shunting Locomotive Officer Z.F. Andarzyanova prepared the receiving route for train #2253 on track #9 and opened the entry signal #N.
After the train dispatcher boarded, the shunting locomotive continued shunting along track #7 in the direction of signal #47 (dwarf), which had a prohibiting signal.
At 7:58 p.m., the shunting locomotive passed signal #47, which had a prohibiting signal, and entered the receiving route.

Violations by transportation personnel:
Shunting Locomotive Officer Z.F. Andarzyanova violated the requirements of paragraphs 2 and 4 of Appendix #9 to the IDP and paragraphs 2.3. The Zyurzya station TRA failed to transmit the order to cease shunting to the shunting locomotive driver via radio and, before opening the incoming signal for train No. 2253, failed to verify that shunting had actually ceased following the locomotive driver's radio report.
Additional information:
According to the report drawn up following the traffic safety incident, the visibility of the prohibiting signal of exit signal 47 is 850 meters, while the PTE requirement requires at least 200 meters from the side track.
The last KMO was held on August 6-7, 2025, under the chairmanship of DS Kuzmin A.V. A total of 100 issues were identified, including 20 issues regarding the signal. There were no issues regarding the visibility of signal 47. Zyurzya Station is equipped with a block relay electrical interlocking system with separate control of switches and signals according to the EC-9 standard solutions, SAUT-C and SAUT-CM devices, and is included in the dispatching interlocking system (DIS) of the "Trakt" system of the 4AD-Druzhinino circle.
Information on the personnel involved:
Zyurzya Station Duty Officer Z.F. Andarzyanova, born in 1972, has 10 years of service (with breaks), including as a DSP since September 9, 2024, and as a DSP at Zyurzya since June 24, 2025. Risk group "unlikely risk."
Zyurzya Station Train Master V.V. Nosov, born in 1972, has 22 years of service (with breaks), including 14 years as a train master. Train conductor at Zyurzya Station - 2 years 9 months, risk rating "unlikely."
Zyurzya Station Master A.V. Kuzmin - born 1972, 25 years of service, including 2 years as a derailment supervisor.

Consequences:
As a result of passing a traffic light prohibiting the derailment of rolling stock, there were no injuries or damage to infrastructure. Delayed: locomotive #7015+0-08 min, 3 freight+01-46 min.
Damage:
ChMEZ diesel locomotive #5957 suffered a fractured automatic coupling device and a damaged track clearer;
VL80S electric locomotive #2244 suffered an oil leak from the transformer unit, damaged automatic coupling device on the head section, damaged batteries, paintwork, and a track clearer. The following personnel were dispatched to the investigation:
